By providing the information set out in this form, I agree and consent to Shangri-La Ltd and its related corporations (collectively, "Shangri-La"), as well as their respective representatives and agents ("Representatives") collecting, using, disclosing and sharing amongst themselves my/our personal data provided above as well as in the records of Shangri-La from time to time, and disclosing such personal data to Shangri-La's authorised service providers, and relevant third parties for purposes reasonably required by Shangri-La and its Representatives to provide me with the services I am requesting for, including but not limited to hotel & restaurant services and third party bookings made on my behalf at my request.
